Privacy Statement and Disclaimer
This privacy notice discloses the privacy practices for any quality improvement project that utilizes the QIDA system. This privacy notice applies to information that is collected in the QIDA system.
System-wide Information Collection, Use and Sharing
At the American Academy of Pediatrics (AAP), we take the issue of privacy and security very seriously. We do not sell, distribute, barter or transfer personally identifiable information obtained from a user to a third party without your consent. De-identified information may be used for the purposes of research across Academy quality improvement efforts. All requests to use data collected through QIDA for research purposes will be reviewed by the AAP Institutional Review Board. The AAP will update or remove collected personal data within the QIDA system if contacted by any quality improvement project participant who is utilizing the QIDA system.
Confidentiality of Data
Data collected will not include Protected Health Information, as the QIDA system does not collect any protected health information within the system. Comprehensive measures will be implemented to maintain subject confidentiality and anonymity as appropriate. Users will use the Academy’s Single Sign On process to access the QIDA system.
Surveys
Some projects request information from project teams via surveys. Information requested may include contact information and demographic information. Survey information will be used for quality improvement purposes or for the monitoring or improving practice processes. Survey data may also be used for research purposes to help stratify quality improvement data within the Academy. Data used for research purposes will only be reported in the aggregate.
Cookies
The AAP may use “cookie” technology to obtain non-personal information from its online users. We do not extract personal information in this process nor do we provide this information to third parties. We also do not contact you based on the information in your cookie file.
Log Information
When you access QIDA, the host servers automatically record certain information. These logs may include information such as your web request, your interaction within the system, Internet Protocol address, browser type, browser language, the date and time of your log-in and one or more cookies that may uniquely identify your browser or your account.
Security
We take precautions to protect your information. When you submit sensitive information into the QIDA system, your information is protected both online and offline. Whenever we collect detailed information about you (such as name and address), that information will be encrypted and transmitted through the system in a secure way. Your email address will not be sold or provided to third parties.
While we use encryption to protect sensitive information online, we also protect your information offline. Only project and QIDA staff that need information to perform a specific job duty are granted access to personally identifiable information. The server that hosts QIDA is kept in a secure environment. All information is protected by our security measures, which are periodically reviewed.
AAP reserves the right to disclose without notice to the user any information in its possession if required to do so by law, to protect or defend AAP's rights or property, or to respond to an emergency. This Web site contains links to third-party Web sites which are not under the control of the AAP. AAP is not responsible for the privacy practices of any linked site.
AAP reserves the right to revise this privacy statement at any time by posting a revised statement on this Web site. Use of this Web site after such change signifies the user's acceptance of the changed terms.
